If this is the first time you have volunteered at Lombard CRC, you are required to complete and return the following two forms:

Also, please note that your email address provided during registration is required in order for our church to conduct a background check.


If you are under the age of 16, you are required to complete the following form:

All volunteers will receive a Lombard CRC shirt. Quantities and sizes are limited. We will try our best to provide everyone a shirt at the requested size, but are unable to guarantee.
